What is a self-declaration (ghoshna patra)?
It is a statement by the applicant that the details given in an application are true, with the applicant accepting responsibility if they are found to be false. Offices commonly ask for it along with income, caste and residence (domicile) certificate applications. The exact format required can differ between offices, so confirm with yours.
How to fill it
- Enter the purpose of the certificate and the Aadhaar number, which prints in twelve boxes.
- Add name, father/husband’s and mother’s names, date of birth, occupation and place of birth.
